Mitch Leyor Model Release Form

I, the undersigned, hereby grant permission to Mitch Leyor and its affiliates to use my likeness in photographs, videos, and other media content for promotional purposes. This includes, but is not limited to, usage on social media platforms, television, websites, and any outsourced platforms.

I understand that these images and videos may be used in various capacities at any time, as long as they remain relevant to the Mitch Leyor brand. This agreement also covers artistic concepts and ideas shared during events related to the brand.

By signing below, I acknowledge that I have read and understood this release form and agree to the terms outlined herein.
